Admissions Checklist
Phase 1 - Complete the Following Steps

1. Visit/Tour the school (New Students)
We have year round open houses every Wednesday from 8 am to 5 pm. We’d love for you to stop by for a visit and let us share FAS and all it has to offer with your family.

2. Submit Application (New and Returning)
A completed application form including signatures of parent and student, and non-refundable application fee.

3. Recommendations (New Students)
Two confidential recommendation forms to completed by principal, teachers, guidance counselor, pastor, elder, and/or Sabbath/Sunday school teacher.

4. Submit a signed Testing Consent Form (for all K-10th)
A face-to-face meeting with the prospective student and a parent/guardian is required with the school principal before the application and acceptance process is complete. Please contact FAS to schedule at a time that is convenient for you. We will do our best to accommodate your request.

5. Interview (New Students)
A face-to-face meeting with the prospective student and a parent/guardian is required with the school principal before the application and acceptance process is complete. Please contact FAS to schedule at a time that is convenient for you. We will do our best to accommodate your request.

6. Official transcript (New Students)
If the student has attended another school, an official copy of the transcript is requested.

7. Finances (New and Returning)
A completed financial agreement form signed by parent/guardian. Several types of financial aid are available to FAS students based on academic qualifications and financial need. Families applying for student aid are required to complete a FACTS Tuition Aid application at https://www.factstuitionaid.com/FACTS/gaapp

8. Emergency Treatment Form (New and Returning)
An emergency treatment consent form signed by parent or guardian.

9. School Questionnaire (New Students)
A school questionnaire to be filled out by parents.
When steps 2-9 are completed, the student’s name will be submitted to the FAS School Board. A letter will be sent
to the student indicating the board’s decision.
The student must have this letter before enrolling.
Phase 2 - Following acceptance, the following documents must be on file with the school office.

1. Copy of Social Security Number

2. Copy of Birth Certificate (all grades)

3. Medical Examination Form
A current physical examination form completed by a physician and obtained no more than six months prior to the student’s first day of school. This requirement is for all new students and students entering Kindergarten, 4th , 7th & 9th grades.

4. Complete immunization records
New Students and anytime it is updated

9. Library/ Textbook Lending Agreement

10. Photo Release
